Please include all the following information in your request:

• Full name used at time of seminar attendance
• License number (for California LAc)
• Year of seminar
• Complete mailing address
• Day and evening telephone numbers
• E-mail address

It will take approximately two weeks for OCOM to re-issue your CEU certificate; there is a $10 processing fee.
